Advance Directive Individual Sessions @ Scotts Valley
Planning ahead for your healthcare is a gift you can give to yourself and those you love. Schedule a one-on-one appointment to complete your Advance Directive with support. You’ll meet with a trained advance healthcare planning volunteer from Hospice of Santa Cruz County to complete your form. You can even get your Advance Directive witnessed to make it a legal document.
